Everything about this is wonderful! I love how the three of them make each other's lives better in so many ways, small and big, and how Holmes is so decidedly Holmes throughout this, casual with his health and guarded with his heart. I adore Mary, and you write her so well here -- she's still Victorian, she's still a product of her time, but none of that keeps her from being so thoroughly decent and spirited and witty and sharp, with an immense capacity for love without being a pushover. And oh, your Watson, he tries so hard and he's so transparent to the people who love him, and that just makes them -- and me! -- love him all the more.
I also really like how Holmes relates to his body, and, indeed, all the different ways that the other characters relates to his body. This story is so warm and rich and complicated, and you write with sympathy and insight.
