(Mostly) Everything before chapter 20 was filler. Start on chapter 20—or don't.
A mentally unstable, delusional Subaru, gets transported to another world.
In this new world, Subaru is constantly making noise about his delusions to anyone who will listen. He's constantly bringing up something called "the fourth wall", something called "Thirth Person", and another delusion named "Readers".
Of course he is ignored by the people around him and is seen as crazy—something Subaru doesn't at ALL like. He takes his delusions very seriously, after all!
Subaru Natsuki is VERY unwell, but profusely denies it.

Summary
In this AU, Subaru and Emilia essentially swap roles in the camp and swap genders. Subaru is female. Emilia and maybe Rem are male. Emilia is a Knight in this AU. Other gender bends will occur as required, such as Satella, Betelgeuse and Sirius, so the story works properly. I made the Subaru a Royal Candidate because I didn't want to rehash the Mimihigau If and thought it would be interesting.
