“My quirk is called Prophecy, and, upon touching someone, it allows me to see their death if they’re going to die in the next hour. When I ran into you on the street, I saw you get shot up here and bleed out before help arrived, so I followed you and pushed you out of the way.”
Midoriya gasps, staring wide-eyed at the girl. “T-thank you! That quirk must be so useful.”
She shrugs and stretches as the scarf loosens just enough to allow her some extra room to breathe. “I don’t use it very often.”
“Why not?!” Midoriya asks again, fully stepping forward beside his teacher.
“Because the universe tries to correct itself, and the person usually dies anyway,” she says bluntly.
Or... Someone with a prophecy quirk sees Midoriya's death and manages to prevent it. Now, she and Aizawa have to prevent the universe trying to correct itself and killing him anyway. Of course, Midoriya can't make it simple when he sees someone in need of help.

He paused. The corner of his mouth twitched as he unfolded it slowly.
It wasn’t his handwriting.
You look tired today. I hope you get home safely. I’ll sleep better knowing you’re okay. Don’t worry, I’m watching out for you. Always.
His breath hitched in his throat as he read.
Or, Midoriya gains a stalker. Aizawa isn't happy.

The deck rocked hard against another strong wave, and Midoriya grasps tighter to the railing, cursing himself for getting distracted as he pitches forward– something wraps around his torso, pulling him back to deck before he’s flung headfirst into the water. Midoriya turns as his teacher’s capture weapon curls back around his neck.
“Careful, Midoriya,” Aizawa says, eyes narrowed. “This isn’t the time to lose awareness.”
“Y-yes, sensei!” Midoriya nods, taking a deep breath of salty air as he reorients himself.
Or, a rescue training exercise on a ship goes horribly wrong when Midoriya slips over the edge...

Crack in the floor–” His throat seized around a cough. “C-Can’t tell how far I fell.” He pressed his free hand to the side of the mess of concrete–what was essentially going to become his coffin if Aizawa didn’t get here soon–and felt along the unyielding metal. His vision pulsed, heart beating too fast and too slow, breathing becoming harder and harder. “There’s no air,” he whispered.
Or, Midoriya ends up buried alive after a villain infiltrates a training exercise. He calls Aizawa.

Summary
Shinsou tensed, started to voice a command to stop, but Midoriya was already swinging his powered fist toward the reinforced wall of the arena.
The impact was thunderous. Cement shuddered, spiderweb cracks racing outward from the point of impact. Dust exploded into the air, the shockwave rattling the ground under Shinsou’s feet. Midoriya’s arm bent at a sickening angle, bone and muscle giving way under the strain of his quirk.
Or, Midoriya trains with Shinsou in order to help with his brainwashing quirk. After an accidental command, Midoriya is injured.

“Leave him alone,” Aizawa says, voice like steel.
The villain chuckles softly, ignoring him. Instead, they tilt their head toward Aizawa, still staring at Midoriya. “Funny thing. He doesn’t make much noise anymore during our meetings. No matter what I try.” He leans closer to Midoriya, lowering his voice conspiratorially. “But he sure was concerned when you didn’t seem to be waking up. You… I can see it already. You’ll scream. And he’ll hate every second of it.”
Midoriya’s breath stutters.
The villain straightens, voice louder now, dripping with cruel delight. “But I’m nothing if not fair. Let’s make it interesting, shall we? Here’s the deal. Every time I decide to have some fun, you, my dear boy, get to choose. Him-” He gestures lazily at Aizawa, whose eyes flash with fury even through exhaustion. “Or you.”
Or, Midoriya and Aizawa are captured by a villain wanting revenge on Aizawa.

Aizawa’s eyes narrowed. “Idiots,” he hissed.
Or, an alternate ending to the Kamino incident when the group rescues Bakugo from All for One. Midoriya gets hurt during the escape.

Midoriya clenched his jaw, forcing himself upright again, even if his body shook with the effort. He scanned the line of classmates who were bruised, bloodied, and had heads bowed. Then his eyes found him.
Aizawa sat apart, bound to a heavy iron chair, arms and ankles shackled in place. A cruel frame forced his eyelids wide open, his usually tired stare now bloodshot and raw. Aizawa didn’t move, didn’t speak, but his gaze swept over the students with steady precision. He was counting breaths. Checking injuries. Tracking.
