Kirk and Spock must help him create a vaccine before the entire crew finishes the incubation period, even though that means keeping McCoy on his feet and working even if it kills him.

Summary
What if Leonard McCoy had died early in the five-year mission? How would the Enterprise’s captain and his relationships with his officers be different? A version of Jim Kirk ends up on the true Enterprise and finds out just how much his life hinged on Leonard H. McCoy.
“Where am I, Bones?” he asked, finding he was almost afraid to ask.
“That’s not an easy question to answer. You’re on the U.S.S. Enterprise, commanded by James T. Kirk. As to where, I could ask the computer for coordinates, but they wouldn’t help. All the more enlightened opinions think you were most likely transported here from another universe entirely. We’ve known it to happen.”
It was a crazy story, one Kirk shouldn’t have believed, yet there was no lie in McCoy’s eyes. Kirk took a moment to process the possibility. “Another universe. Then when I saw myself –”
“You, but not you. It really is a mind trip, isn’t it?”

Kristen scoffs. “Oh, what, it’s just gonna go away one day? I just have to survive until it does? I don’t want to,” she insists. “I don’t want to sit around and wait for things to feel better. I don’t. I can’t.” She shakes her head rapidly, jostling the tray on her lap, spilling bits of liquid down the side of the soup bowl.
“Are you saying—”
“No,” Kristen says quickly. “I’m not going to off myself. I just… I can’t believe it. I can’t believe that I’m alive and Fabian isn’t. It’s not fair. It’s my job to keep him alive and… and I didn’t. Lydia, I didn’t.” Tears form in Kristen’s eyes, spilling out faster than she knew possible. “It’s my fault.”
