The fluid from the rat hadn’t been blood.
It was a parasite, and it had found its host.

Summary
“Sam?” he calls again, reaching into his waistband and pulling out a pistol. He sees the bathroom door is open a crack, the lights on. He steels his nerves and takes slow, silent steps to the bathroom.
He pauses outside the door, listening intently. He doesn’t hear anything, so he flings the door open and steps in, gun aimed around the edge of the frame.
The sight that awaits him is the worst thing he’s ever seen and ever will see, he’s sure of it.
“No!” His voice breaks along with his heart, his mind, his soul-
Sam is in the tub, and the tub is covered in blood. He’s in the same t-shirt and cargo pants he was in when Dean left, but now they’re soaked in wet, hot, sticky blood.

Or, Sam's been struggling lately, and it's affecting Dean more than either of them seem to realize.

“You’re kinda nice, Dean,” he says, and Dean’s mouth falls open. Sam giggles and lifts a shaking hand to point at it. “Your teeth are showing.”
Dean huffs out a quiet laugh. “Oh, man, they got you on the good stuff,” he says. Sam responds with another giggle, and Dean shakes his head fondly.

Summary
When Dean finally gets them out of the stuffy bar, he’s sweating bullets and his arms are aching from holding up his brother. God, Sam’s gonna owe him for this one.
Sam’s knees buckle a few feet away from the Impala, and Dean barely catches him before he hits the ground. “Deeean,” Sam whines as Dean tugs him up again.
“Shut up,” Dean mutters, leaning Sam against the car to open his door. Sam teeters dangerously to the side and Dean has to grab him again so he doesn’t end up on the concrete. “Dude, you’ve never been this drunk before,” he says. “Fucking hell.”

Or, Dean finds Sam wasted alone at a bar and hauls him home, terrified when his brother starts showing signs of alcohol poisoning. But did Sam really just overindulge that much? Or is something else going on?
