Chad's been obsessed with this Dean Washington guy ever since he showed up a few months back. According to Chad, Dean looks like he could be Jensen's older, hotter identical twin (hence, dopplebanger – "it's like a doppleganger, but instead-" "yes, Chad, we get it."). Frankly, Jensen's not even sure how Chad came up with that idea, since most days, Dean rolls in looking like a half-feral hermit who's gone 15 rounds with a combine harvester and lost. Now, they get a lot of veteran visitors at St. Anthony's and gnarly scars are nothing new, but it's clear that whatever happened to Dean had to have been hellacious, and Jensen's not one for prying into someone else's personal business.
