Contrary to general belief, Letho of Gulet does have feelings. Has an abundance of them, in fact. Most Witchers do. So his problem isn't a lack of emotions as much as it is a lack of knowledge of what the hell most of them even are and what he's supposed to do about them when he's got them.

Geralt chooses to divert from the prison barge escaping Flotsam, and Iorveth chooses to save him from drowning in the filthiest river spanning The Continent. Of course, neither of them realize that this little detour will cost more than they could ever hope to afford.

Summary
Based on the game choices where Dettlaff killed Syanna and Geralt lets him go. Five years later, after Regis has been putting a lot of work on Dettlaff, invited him to a performance Dandelion and Priscilla wanted to share. Some don't know he's a vampire and thus makes everyone uneasy. Regis knows that Lambert likes Gwent and to diverge attention and tension, suggests that the two witchers play a few rounds. Leading to some interesting bonding moments.
