Dazai generally doesn't care about his physical appearance, he has better things to think about than something as insignificant as his looks, however, there is something that perhaps generates annoyance but it's not a big deal according to him (it really is).
he had been analyzing (over thinking rather) what he told "you could look better" Did Chuuya consider he looked good, Dazai had never looked like someone good looking, his hair has always been a mess, his weight is too low for someone his age, he didn't have a pretty face, his attitude wasn't the most attractive and the most obvious, he is bandaged from head to toe, possibly giving competition to some mummy.
