Post Season 3 Finale, Daisy never left, and Coulson stayed as Director. Things continued on as normal, despite their previous losses nagging constantly in the back if their minds. Daisy had been struggling since Lincoln died, and the newest mission didn't help. She'd been injured. An unseen brain bleed. She'd begun to see the people she's lost, good and bad. Things were taking a turn for the worse, and her end was inevitable by the time anyone caught on.
TLDR: Daisy sees ghosts after an injury.
