Sam has started the Apocalypse and the guilt consumes him. He's broken by Dean's abandoning of him and wonders why he's still around.
Season Five Episode 3 (and the beginning of episode 4); I was having a lot of feelings about Sam's arc and how it relates to Dean. I did not finish episode four before writing, so it's canon compliant to the point of Sam calling Dean about Lucifer. After that, the stress gets to Sam, and he doesn't see the point anymore.

Summary
Spots dance across Sam’s vision as this thing chokes the life out of him, and for a second, Sam freezes. For the first time in his life, he hesitates to yell for Dean. He keeps his mouth shut as his vision starts to tunnel, as the breath he should’ve used to scream is ripped from his lungs.
Sam is struggling after separating from Dean, and even more so after reuniting. But why does it matter? Dean's going to kill him anyways.

“Cas! Cas, please! Please, answer me! Cas!”
Castiel ignores Dean for several minutes, but then Dean gives him an opening that might help him complete his mission, so he goes to investigate. What he finds is a very bloody, nearly dead Sam and a frantic Dean trying to stop the bleeding. Dean tells him where the injuries came from, and Castiel quickly becomes confused. It doesn't make sense, but Dean tries to explain it, and slowly... Castiel begins to understand.

Sam chalked it up to habit. He figured, despite the friction between them, Dean was still bound by childhood instincts. Some things he could override, especially when he wanted Sam dead, but some things remained under all circumstances. So, when Sam woke up in the middle of the night and bolted for the toilet, it really shouldn’t have surprised him that after a mere ten seconds of retching, his hair was being pulled back.

“Sammy,” Dean whispers. “Tell me you weren’t doing what I think you were.” He tucks the gun into his waistband and grabs Sam by the shoulders, trying to meet his eyes.
“I’m sorry,” Sam says again, tears falling freely now. “I should have let you do it, but I can’t take it anymore. I’m not strong enough.”
“You should have let me what?” Dean asks. He locks eyes with Sam, and for once, Sam can’t find any hatred or disgust, just fear. The look on his face is practically begging Sam not to say it.
“Kill me.” The words ring out in the dead of night like the gunshot that almost was.
For once, no words come to Dean immediately.
“Like you said,” Sam prompts, as if Dean needs a reminder. Dean still stares at him, horrified, confused.Series
