Basically where John "Soap" MacTavish is in The TortureChamberVerse, he's being ground up, (aka brainwashed and having amnesia, never quite remembering his close or distant faces). Every place feels vaguely familiar, but at the same time, nothing ever felt the same. Vladimir appears further in the book for the sake of the plot.
John didn't know what was going on.
