Really fucking good, both in writing and in how it creates the of sharing your body with something horrible that's slowly overtaking you. Also does really well with using sex scenes to convey the fear of that thing hurting the person you love mixed with the desire to still be close to them and find comfort in them because you feel like you're losing yourself more without them. Making Yuuji not remember swallowing the finger and thus not knowing who or what Sukuna is really elevates it, and how it shows us through Yuuji's limited POV that Megumi is keeping an eye on him looking for signs of Sukuna while going to Jujutsu Tech and not telling Yuuji what the school actually is for + Yuuji not telling Megumi that he's not alone in his own head... So well done.
Maybe this comes off differently if you find these things hot but this is solidly in the horror camp for me. I still respect it if it was made to be both though (sometimes great horror is someone else's kink etc etc)

Suguru catches himself on his hands. He heaves and gags, lungs spasming at the taste of the curse. Bile and water splatter onto the gravel... This is humiliating.
Something bumps against his shoulder, a water bottle. He glances up at Satoru, who’s very pointedly looking away.
“You know, my six eyes are killing me too.”
Suguru raises an eyebrow. What?
OR: Suguru isn’t the only one hiding the toll that being the strongest takes on him. Turns out that's a bonding experience.

I really like how people run with the vaguely implied negative side effects of satosugu cursed techniques and this one does it particularly well. The caretaking is very sweet and the way it approaches how they react to the vulnerability and why feels very thought out/tonally cohesive with canon

“Cursed Spirit Manipulation?”
It’s the second thing Satoru notices, right after the bangs.
His new classmate is a lot to look at, and all of it neat. His weird uniform is neatly pressed. Neat custom collar that Satoru wants for himself. His hands are neatly tucked behind his back and his hair is neatly secured in a bun. Hell, even his smile is neat—literally everything about this guy is neat—except for the bangs.
The bangs stand out. And so does the technique.
Stepping into his new homeroom seven or eight minutes after the start of the school day, Satoru peers over his glasses at the swirling mass of amalgamated cursed energies at Geto Suguru’s center and concludes:
“Man, that’s ass.”
Satoru makes a bad first impression.

Really in character and there's no way they didn't get into a fight the first day they met and then immediately declared themselves friends after. It just feels right. Also Gojo being extremely autistic but obviously not knowing that and blaming it on the Six Eyes also feels very fitting lol

Summary
“It doesn’t hurt.” Satoru answered the unspoken query. The tenderness of Suguru’s touch when he grazes a thumb across the lines and the wry smile he shot at him draws up an indecipherable well of emotion that closes his throat, prompting an involuntary swallow.
