But Dean's 21 now, and his body suddenly rejects the suppressants in the middle of his work shift, causing him to black out.When he comes to again, the life he once knew is gone.

Castiel is a caregiver.
He fosters Littles who need a temporary place to call home and someone to act as their Guardian until they can be adopted into their forever home.
The fostering agency thinks Castiel would be a perfect fit for a new client of theirs, and once Cas hears Dean Harrison's story, he can't say no.
It's going to be a slow process, but Cas is sure he can help Dean accept who he is and help find him the perfect Caregiver to adopt him at the end of it.But maybe, just maybe, the perfect Caregiver for Dean was right here, all along.

This was created from a snippet posted on my splatter board. After Mary died John fell deep into the bottle. During this time he meets Kate, who doesn't really like Dean. Dean was kicked out of his home when he was just about to start high school due to his now useless status of older brother. Now three years later he's being tested for his classification. He did not know what he was going to become but he prayed that whatever it was...would get him off the streets and in a better place. Classified as a little, Dean's now faced with a whole new world that's terrifying and even more scary with John's voice in the back of his head. Dean's Papa and Dada trying to put the pieces back together for their little boy while giving him everything he could ever want. Dean was just doing his best to protect his pain in the ass little brother from the monsters in the world, he didn't want any of this. Not the oddly huge clothes he didn't remember putting on, or to be stranded in some tiny city he'd never been to before. He definitely wasn't expecting the giant guy that was chasing him around spouting gibberish about being his little brother. Yeah, no, Sammy was puny not this gigantic old man with bad hair. Then there’s this guy that claims to be an angel, as if those were real, with his huge blue eyes and the way he treats Dean like he’s actually worth something, like he’s someone who deserves to be cared for. No, Dean didn't ask for any of this... but it might not be all bad.
