He isn't stupid. He’s tried to stop this "fugue state" from happening. He has a good handle on it, most of the time. Yes, he carries a stuffed animal everywhere with him and sure, he sometimes forgets he has a quirk, but it's all entirely under control. Really.
As for Kacchan, well. He’s been strangely understated about the whole issue. Suspiciously so. Izuku isn’t one to push back against acceptance, though, no matter the circumstances, so he leaves it alone.
If it were a problem, Kacchan would have told him, after all.
