She’d put Arthur to bed, their current case was in a security-footage watching lull, and Sherlock had yet to hit the slightly manic and fidgety stage of their late-night digital stakeout. This was as good an opportunity as Joan was going to get to ask Sherlock what she needed to ask him.
“Well that doesn’t instill a tremendous amount of calm in me, Watson.”

Which is why Sherlock's sudden desire to have Joan preen his wings is so confusing, because it's not like that. But dismissing the thought has yet to work.

“What’s your name?” he asks, because if their patient is conscious, he might as well tell them. There’s something uncomfortably detached about referring to someone as “that guy” when you’ve seen half of their innards spilling out. “I’m Sanji,” he offers when he gets no immediate response, and he itches for a cigarette to entertain himself with while the man pulls his feet under him like a newborn lamb.
In the faint moonlight and backlit with the light from the hallway, he’s both deathly pale and bathed in shadows that make him look gaunt and distraught. His hand reaches up to his throat, not touching, just hovering, and he opens his mouth on a word, his lips dry, pink with small cracks that threaten to break and well up with blood. No sound leaves him.
His hand drops, mouth closing with a clack of teeth gnashing together, swallowing down ire instead of words.
Sanji frowns. “Can you talk?”
His stomach drops at the glare that earns him. Vicious and a little wild around the edges, like he’s mad Sanji even dared to ask.

He took a deep breath and braced his abdominals before cautiously drawing Watson out of the fireman’s hold over his shoulder, and now he could feel the chill dampness exposed on his back, where she’d been. He attempted to set her on her feet, willing her to stir, to resist his support, react somehow to the ignominious treatment of her person. Her ankles and knees immediately gave way and he tightened his grip to ease her all the way down to the ground. “I really don’t know what I did to deserve this cold shoulder,” he said to her. “And I shall hold you responsible for the indignity of that poor excuse for double entendre.”
