The Shadowlands were the worst for so many reasons. The sun was gone, there was nothing to eat, and it was absolutely freezing. Astarion was used to the cold, with 200 years of vampirism under his belt. The lack of a normal circulatory system meant his body couldn't regulate temperature the same way living creatures did. A vampire could be frozen stiff in minutes before they’ve even noticed there was a problem. He could attest to that. And no additions of hats and scarves made a bit of difference - if there was no heat in the first place for the clothing to keep in.
It really was bitterly cold.
