Nightmares have always haunted Dean. First it was his mother's death, then when he starting hunting he had different nightmares. After hell and purgatory, he's got lifetimes of nightmares.
These new ones are different. These new ones, he sees every time he closes his eyes.
They defeated Chuck, installed a new God, and now Dean has to live with the aftermath.
He doesn't know if he can.

His vessel was starving. No matter how many burgers Castiel ate, he couldn’t get full.
There was something else too. A drive inside of him, a hunger that wasn’t physical. It was a hunger that was growing below his stomach, an ache that he needed to fill.
What was this feeling? Castiel had never felt anything like it before.
That was a lie. He was pretty sure he knew what it was. It was lust. It was lust for a certain green-eyed man who Cass wanted to devour.

Dean and his dad hit a bar after a hunt. His leg’s bleeding, Sam hasn't been picking up his phone in weeks, and John’s halfway to drunk out of his mind. So far, so usual.
What’s unusual, though, is an old hunting buddy of John’s showing up out of the blue, and not just because he’s in full cowboy getup in a seedy bar.
Over the course of one evening, he starts making Dean question everything he thought he knew about what it means to be a hunter.

Bobby was right: this is the Heaven Dean deserves because it’s worse than Hell. He knows what Castiel is asking him to say. He’s practically begging Dean to say it and it’s the one thing Dean can’t say because if Dean says it out loud and he’s wrong...
The status quo hurts but Dean would rather keep things the way they are than be wrong. He’d rather cease to exist than spend eternity in Heaven if he’s wrong. “Don’t you know, Cas,” Dean pleads.

Dean can't allow himself the luxury of getting attached.
So when he spends the best night of his life with a man who calls himself Emmanuel, he knows right away that nothing more will ever come of it. It doesn't even matter that Dean's unable to really forget the guy many weeks and months later, that is just the life.
But then Sam ends up cursed on a hunt and after a long and ultimately futile search they find themselves with only one option left to save him: the healing abilities of an angel.
Namely, the Fallen One.
Castiel.
Dean's never heard of the guy before, but if he's really capable of curing his brother, so be it.
He is in for a rather big surprise, though, when he suddenly sees himself confronted with a very familiar face and soon enough his life is turned upside down in a fairly inconvenient manner.

(In which Dean realizes he slept with an angel of the freaking Lord, has a major freak-out about it, and then, to his own chagrin, can't stop thinking about doing it all over again as he and his cursed brother end up cohabiting with the angel in question.)
