You weren't surprised when you woke up in the pod - you always did. After every victory or death or failure, you woke up right as that mind flayer forced that tadpole into your eye. You killed the chosen three an infinite amount of times, destroyed the elder brain more times than you could count - died even more! And yet, no matter what you did, you always woke up at the very beginning.
A lonely existence, for anyone, having to re-establish eternal loves and undying friendships all over again. It felt like a hopeless cause, having to play the same game over and over again. Resetting no matter how it ended. You thought it was an eternal, lonesome torture.
Until you find out a certain devil is suffering in the same hell.

A large hand closed around the vampire spawn’s ankle. He was on his ass in an instant with Caspian rising enough to try and shove a knee on the smaller man’s gut to keep him down.
“Rude, lying, knife-wielding assholes don’t get to make demands,” Caspian growled before he caught a slash across his face for his troubles.
or basically what would the game be like if Tav was a giant, traumatized, half-elf barbarian werewolf who forces everyone to be nice and eventually melts the cold, dead heart of a certain vampire.

Astarion is enjoying married life in Baldur's Gate when he discovers his parents are still alive. Alongside his wife, Tavya, he travels to Evereska to meet them - but family reunions are seldom simple.

But after the sting of your heads colliding, this strain is too much for your mind to bear. Your vision teeters, hanging on by a single thread, all that’s left are that man’s intense red eyes.
You emerge from your trance with a shaky breath. You reach for your throat, as if to make sure that it’s still in one piece, only to find it coated in a thin sheen of sweat.

You saved Baldur’s Gate almost 300 years ago. You died 150 years ago. On a new life now, you find that memories from your past lead you to a specific silver-haired man. Who was he, and why won't he leave you be?
tldr; An Elf-Tav reincarnation story where Tav dreams about Astarion in their nightly reveries and eventually seeks him out once they reach maturity. Things definitely totally go well.

Summary
Fifty years had passed since the Netherbrain was defeated; it took Astarion five years after to become the lord of Baldur’s Gate. Within that timespan his old companions departed from the city, following new adventures in quiet hope their paths would never cross again. Only one agreement was made among them, set by the leader themselves that carried the entire team on her back single handily.
“Keep it within Baldur’s Gate.”
A warning, no less, a boundary they set to ensure the entire realm wouldn’t be overrun by vampires, although Astarion would be lying to say the thought didn’t cross his mind every other day or so.
The temptation was always there, and it would be so easy.
As much as Astarion would never admit it, he did respect his dear old friend more than he let on.

After a forfeited university scholarship and four wasted years with a woman who ultimately stranded her and left her for dead on a deserted stretch of Amnian highway, Miriam is determined to finally get her life back on track. What she didn't plan for was finding out the attractive stranger she's been anonymously hooking up with the week before classes began would turn out to be her physics professor for the semester.

Tav is about to try to convince her it’s nothing, perhaps blame the exhaustion, maybe play it off as fever. He doesn’t get to, because another blade is added into the equation, slipped around Lae’zel so quickly and softly that neither of them notice before the owner speaks.
“You so much as twitch, gith, and I will cut your head clean off.” Astarion hisses into her ear. “You think he is the only one watching over me obsessively? It goes both ways, darling, we are never out of each other’s sight. Now drop the fucking sword.”
Lae’zel is frozen still for another moment before she curses lowly in her throat and drops her weapon, her hands slowly raising in surrender.
Astarion, defending him against a githyanki and making her bloody submit to him. Gods, Tav wants to kiss him stupid.
