Remus frowns. “I didn’t think there was much else at all, what with all the inbreeding.”
There’s a quiet thud as Regulus drops his head onto the sink counter. “Get out,” he says, pointing at the door with as much absolution as he can muster. Remus only laughs.
Something is wrong with Regulus Black. He’d hoped leaving home would fix it, but he’s staying at James Potter’s house for the summer, and things will only get more complicated from there.

Summary
Regulus Black has been named heir after his brother fled from Grimmauld Place. It is the only reason his parents accept that he is trans. He has inherited all of Sirius’ responsibilities and expectations. His secret relationship with James Potter has ended and on top of that he finds out he is pregnant. He leaves Hogwarts, Sirius and James believe he has been sucked into Voldemorts wrath but little do they know Leo has been born.
Several years pass, the news of Regulus Black dying has been and gone. Sirius has learnt how to live with his failure as a big brother. He learns how to live in a world without his baby brother, until one day the dead comes to his front door with his son in tow.
THIS IS NOT MPREG. This is a female to male trans character getting pregnant, deciding to keep the child and raise it. There are no details of the pregnancy or childbirth as there is a huge time jump to when the child is older.

After avoiding the Black family tapestry, Sirius finally faces it. He reasons with himself to avoid false hope, but after confirmation from another source he confirms his brother isn't dead. He also finds out some more interesting facts, and it turns out Regulus was smarter than he thought.

Although the villain seems to be treating the hero's sidekick better than the hero ever has and they start to become fond of each other. Although this fondness has leaked into the hearts of the actors and the interest has become more than acting.

He knew he must've been staring too long— focused on how nice her hair looked pulled back by a bandanna with a few loose strands hanging out— when Evans looked up to meet his eyes and, after a brief moment of consideration, smiled at him. Lily Evans was smiling at him.
Regulus gave a polite smile back, looking down at his half-finished plate of food after staring at her one moment too long. Regulus tried to ignore how his heart was beating faster than before, how flushed his cheeks were and how he had to fight to get his expression to fall neutral once again. He had to ignore it, suppress it, because that feeling reminded him of what he had felt for Potter, back when he was a naive first year. How desperate he had been to impress James, telling himself he only wanted to do so because he was one of his brother’s closest friends.
But his feelings for Potter had never really gone away in the first place anyway, had they?
Or; Regulus is back for his fifth year at Hogwarts, lonely and resentful. He doesn't expect to get wrapped up in a scheme to get his estranged brother and werewolf crush together. He certainly doesn't expect falling in love with a blood-traitor and a Muggle-born along the way.
